WThe referral form was developed according to /Attachment. No. 3a / (Journal of Laws of 2015, item 457) by the MEDYK Medical Center. Only employers authorized to use them have an agreement with the MEDYK Medical Center for performing preventive medical care over employees.

Examinations of underage patients up to 18 years old take place in the presence of parents or legal guardians. In the case of underage patients between 16 and 18 years of age, to carry out the examination or surgery, in addition to the consent of the parent or legal guardian, the consent of the minor patient is also required.
